President Joko Widodo has officially issued Regulation of the President No. 59 of 2024 on the Third Amendment to Regulation of the President No. 82 of 2018 on Health Insurance (“RP 59/2024”). Under this regulation, several changes have been made to the BPJS system, specifically the dismissal of the class system in BPJS and the adjustment of BPJS members’ fees. This regulation also introduces the Standard Inpatient Class System (Kelas Rawat Inap Standar – “KRIS“), which hospitals are expected to implement in cooperation with the Social Security Agency for Health (Badan Penyelenggara Jaminan Sosial Kesehatan – “BPJS Kesehatan“) by no later than 30 June 2025. Further provisions that specifically address benefits, tariffs and fees will be determined by no later than 1 July 2025.
The implementation of the KRIS system is set to replace the current BPJS Kesehatan system of Classes 1, 2 and 3. The new regulation requires all treatment room facilities used for KRIS-based services to meet specific criteria, including: 1) Building components must not have high levels of porosity; 2) Must have good air ventilation; 3) Rooms must be well lit; 4) Must have bed equipment; and 5) Bathrooms must meet accessibility standards. BPJS members will receive the same service for standard treatment rooms. This KRIS system will be comprehensively implemented in every hospital that cooperates with BPJS by 30 June 2025, in accordance with the terms in Article 103B paragraph (1) of RP 59/2024.
BPJS members’ fees will be standardized into a single rate, with the amount still under discussion. There is a possibility that BPJS members who previously were in the first and second classes will have their rates reduced, while rates for third-class members might increase.
The Indonesian Minister of Health stated that this new system is being implemented to provide better and more equitable services. Coordination with hospitals, BPJS Kesehatan, as well as the Ministry of Finance will ensure smooth implementation of this system.
